Requirements: Valid Nursing License, BLS/CPR, valid driver's license, and current auto insurance
The Registered Nurse (RN) provides direct patient care to hospice patients and supportive care to their families. The RN manages assigned patient clinical records and works with an interdisciplinary team consisting of LPNs/LVNs, CNAs, physicians, and other support staff to ensure high-quality hospice care.
ResponsibilitiesProvide and document the highest quality of care according to the Plan of Care.
Perform initial and ongoing patient assessments.
Develop and implement the Hospice Plan of Care in collaboration with the physician and interdisciplinary team.
Provide physical, psychosocial, educational, emotional, and spiritual support to patients and families.
Report changes in patient condition to the Attending and Hospice Physicians.
Maintain accurate and updated medical records, including assessments, Plans of Care, and interdisciplinary notes.
Follow infection control measures and standard precautions.
Monitor staff scheduling and participate in the Quality Assessment and Performance Improvement Program.
Participate in the agency on-call schedule when required.
Maintain knowledge of pain and symptom management for terminally ill patients.
Perform routine supervisory visits for other team members at least monthly.
Review and follow up on patient clinical records to ensure quality care, regulatory compliance, and adherence to policies and procedures.
Maintain communication with patients' families to provide information, support, and continuity of care.
Serve as the primary liaison between physicians, patients/families, and the hospice team.
Educate patients, families, and caregivers on medication administration, dressing changes, oxygen administration, hospice philosophy, and care of terminally ill patients.
Attend staff meetings and complete required compliance training.
Perform other duties as assigned.
Current RN license in the state of employment.
Current BLS/CPR certification.
Valid driver's license and current automobile insurance.
Minimum 2 years of RN experience in acute care, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered on a case-by-case basis.
Hospice or home health experience is preferred.
Strong patient assessment, documentation, communication, and care-management skills.
Knowledge of hospice care, terminally ill patients, and family support.
Knowledge of Medicare, Medicaid, ACHC, managed care principles, and applicable regulatory requirements.
Ability to work effectively with an interdisciplinary hospice team.
Strong organizational and program management skills.
