The Care Team Hospice is seeking an HCHB Integration Application Specialist to support, optimize, and enhance our Homecare Homebase (HCHB) and related application systems across our growing organization. This fully remote role is ideal for someone who enjoys problem-solving, system optimization, user training, and workflow improvement in a fast-paced healthcare environment.

Schedule & Work Environment
• Fully remote position
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM

Key Responsibilities
• Support and optimize Homecare Homebase (HCHB) and related healthcare applications
• Assist with system configuration, implementation, upgrades, and release management
• Troubleshoot complex EMR/EHR issues and identify root-cause solutions
• Support reporting requests and workflow improvements across departments
• Partner with users to improve operational efficiency and application performance
• Provide education, training, and ongoing support to application users
• Participate in merger and acquisition integration efforts as needed
• Collaborate with vendors and internal stakeholders to support system success

• Bachelor’s degree preferred
• Two or more years of EMR/EHR application support experience within Home Health and/or Hospice
• Experience supporting healthcare applications such as Homecare Homebase (HCHB), Forcura, Medline, or similar systems required
• Experience with EMR/EHR implementation, configuration, troubleshooting, and user support
• Strong understanding of healthcare workflows, including clinical and back-office operations
• Ability to manage multiple priorities, projects, and deadlines in a fast-paced environment
• Experience with reporting, system upgrades, and process improvement strongly preferred
• Strong communication and problem-solving skills
• Ability to pass a criminal background check
