Job description

SUMMARY Provides initial evaluations, crisis intervention and symptom stabilization, emergency evaluations, care coordination, consultation and education to consumers of all ages who present with behavioral health needs. Services will be coordinated with family and other professionals or concerned parties and may be performed in a clinic location, in identified community sites in the region, and in homes. RESPONSIBILITIES Provides Screening Evaluation on the Telephone and Face to Face. Conducts biopsychosocial assessments in agency facilities, community locations, and homes, assessing for risk of harm to self or others, using appropriate assessment tools for accurate diagnoses and case formulation. Completes crisis assessments according to required timelines, consulting with family, friends, police, physicians, treatment providers as needed.

Recommends disposition and plan for stabilization and offers treatment options and recommendations that are least restrictive alternatives appropriate to level of risk. Facilitates involuntary/voluntary hospitalizations of consumers at risk while at the ER and in the community. Provides emergency case management for consumers until they are no longer in crisis.

Identifies and offers referral assistance and coordination to consumers who do not meet criteria of Gateway Healthcare, Inc. treatment programs. Schedules and participates in psychiatric/medication evaluations with psychiatrist as needed.

Maintains current job knowledge of all agency programs, their functions and is able to access these programs effectively. Coordinates and consults with ER staff while performing an intervention and acts as liaison between the hospital and referral resources. Ensures that crisis contacts with consumers involved in the agency is communicated to the primary clinician and physician.

Participates in treatment planning meetings and difficult case conferences when necessary for specific consumers. Adheres to documentation standards in accordance with Gateway Healthcare, Inc. policy, procedure and licensing standards.

Obtains past records in order to substantiate and/or support diagnoses for treatment. Coordinates with Registration Staff on insurance verification and completion of required paperwork, including Releases of Information. Schedules duties in coordination with other ES team members and modifies work schedule as needed to ensure that the department is able to be responsive to consumers needs during all open facility hours.

Appropriately responds to incoming calls on National Suicide Prevention Lifeline by actively engaging the caller while also functioning as a resource to the caller: clinician will assess for risk, coordinate referrals and/or emergency services and completing appropriate documentation of each call. Is reliable and available by pager and/or by phone during the performance of duties. Responds to all requests for consultations/evaluations by the staff in other departments and/or from our community partners (i.e

local medical providers, family, police) and assists as needed in coverage at other ES locations if staffing necessitates and/or supervisor's discretion. Participates on GHI Community Disaster Team. This includes assessing the need for a team response, calling team members to organize and activate a response, and notifying management according to Disaster Team Response Policy.

Participates in all necessary meetings and trainings as required. Performs other duties as assigned. MINIMUM QUALIFICATIONS Master's degree in counseling (MA), MSW, MFT; LCSW, LMHC-A, LMFT-A or LICSW preferred.

Must be eligible for "Qualified Mental Health Professional" status. Eligibility for child & family competency. At least 2 years' experience working in a behavioral health setting.

Basic life support certification. SPECIAL REQUIREMENTS Knowledge of human development and family systems issues. Valid driver's license.

Automobile insurance coverage. Ability to work collaboratively in a team. Flexible work schedule for some evening and weekend coverage.

Strong oral and written communication skills, including speech, grammar, spelling, and legibility are at sufficient level to ensure effective and appropriate communication. ETHICS Uses and accesses information in client's health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc.

policies, rules, and procedures as adopted by Gateway Healthcare Inc. Comply with agency ethics and codes of conduct in all contacts with clients, co-workers and the community. Observe all agency policies and procedures.
