Job DescriptionJoin an amazing team today! Saint Luke's Home Care and Hospice is seeking a full-time RN Clinical Intake Coordinator!
  • RN, MO and KS required

  • 4-10-hour shifts; 8a-6p

  • Every Saturday and Sunday required; remote

  • experience in home health or hospice preferred

The Opportunity

The Clinical Intake Coordinator, RN serves as the clinical expert within the Intake department, ensuring referred patients are screened evaluated, and admitted to Home Health and Hospice services in accordance with Medicare, Medicaid, commercial insurance requirements, and organizational policies.

This position combines strong clinical judgment with excellent communication and critical thinking skills to coordinate timely referrals, remove barriers to admission, collaborate with referral sources and interdisciplinary teams, and support exceptional patient care from the very first point of contact.

The Clinical Intake Coordinator works in a fast-paced environment requiring independent decision making, prioritization, and the ability to manage multiple referrals simultaneously while maintaining regulatory compliance and outstanding customer service.

Primary Responsibilities

  • Review and clinically evaluate Home Health and Hospice referrals for appropriateness and eligibility.

  • Coordinate referral processing from hospitals, physician offices, skilled nursing facilities, clinics, and community providers.

  • Identify missing clinical information and obtain necessary documentation to support admission.

  • Interpret Medicare, Medicaid, commercial insurance, and agency admission requirements.

  • Collaborate with physicians, case managers, referral sources, clinical liaisons, schedulers, and operational teams to facilitate timely patient admissions.

  • Communicate referral acceptance, pending status, or inability to admit with referral sources and patients when appropriate.

  • Support insurance authorization and appeals processes as needed.

  • Assist with regulatory and compliance-related workflows, reporting, and quality initiatives as needed.

  • Utilize Epic and other clinical systems to accurately document referral activity and maintain complete medical records.

  • Prioritize urgent referrals while maintaining established service standards and turnaround times.

  • Participate in process improvement initiatives to enhance referral efficiency and patient access as needed.

  • Provide clinical support, training and guidance to Intake team members and other operational departments.

  • Maintain confidentiality and comply with HIPAA and all regulatory requirements.
