At Maple Farm, our CNAs are caregivers, motivators, and problem-solvers. Our CNAs support residents with daily needs while sharing laughs, stories, and small moments that make a big difference.
What You'll Do
  • Comply with established policies, procedures, and standards, including the Compliance Program/Code of Conduct
  • Follow Residents' Rights policies at all times
  • Maintain confidentiality regarding all resident information, conditions, and family matters
  • Follow written and oral directions, performing duties according to nursing service policies and procedures
  • Assist new employees in understanding facility policies and procedures
  • Identify resident problems, concerns, or changes in condition and report immediately to the charge nurse
  • Complete assignments timely, completely, and accurately while respecting residents' preferences
  • Conduct resident rounds as assigned
  • Identify safety hazards and emergency situations, initiating corrective action or seeking assistance from LPN or RN if needed
  • Report incidents and unusual problems according to facility procedures
  • Participate in the development of individualized care plans for assigned residents
  • Document care and treatment provided in a timely and accurate manner
  • Follow procedures for resident admission, discharge, and transfer
  • Respond promptly to call bells and ensure residents have call bells within reach
  • Assist residents with activities of daily living, including meals, nourishment, and transportation to the dining room
  • Collaborate with the interdisciplinary team to provide consistent, high-quality care
  • Observe all facility safety policies, including the injury-free lifting program
  • Maintain a clean, neat uniform and professional appearance
  • Demonstrate dependability and punctuality
  • Assume Quality Assurance duties as assigned
  • Follow infection control procedures
  • Ensure the safety of residents under your supervision
  • Assist with transporting residents to activity programs and scheduled events
  • Provide activities for residents, conducting programs, visits, and activities tailored to residents' interests and abilities
  • Document activity participation and responses in PointClickCare
  • Perform other duties and responsibilities as assigned

What We're Looking For
  • Meet training and competency requirements as defined in 42 CFR 483.75
  • PA certification through an approved training program and competency testing within 120 days of hire
  • Listed on the Commonwealth of Pennsylvania's Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation of resident property
  • Treat residents with kindness and respect
  • Strong organizational skills with the ability to prioritize tasks
  • Knowledge of computers, fax, telephone, copier, Nurse Call System, Security and Fire System, and PointClickCare documentation systems
  • Must work scheduled shifts, including weekends and holidays as assigned

Additional Information
This position is full-time 3rd shift (11:00pm-7:00am) with every other weekend!
