Physical Therapist (PT) - PRN / Full-Time

Pleasant Acres Rehabilitation & Nursing Center

Join Our Growing Rehabilitation Team

Pleasant Acres Rehabilitation & Nursing Center is seeking a compassionate and motivated Physical Therapist (PT) to join our dynamic rehabilitation team. We are currently hiring for both Full-Time and PRN opportunities. This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes mobility, independence, functional improvement, and enhanced quality of life.

Responsibilities
  • Evaluate residents and develop individualized treatment plans based on clinical findings and resident goals.

  • Provide skilled physical therapy interventions focused on improving mobility, strength, balance, coordination, and functional independence.

  • Monitor resident progress and adjust treatment plans as needed.

  • Accurately document evaluations, treatments, progress notes, and required clinical information.

  • Collaborate with interdisciplinary team members to support resident care goals and discharge planning.

  • Educate residents and families regarding treatment plans and safety strategies.

  • Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.

  • Participate in care conferences and team meetings.

  • Ensure a safe and therapeutic treatment environment for all residents.

Qualifications
  • Graduate of an accredited Physical Therapy program required.

  • Current Physical Therapist license in the State of Pennsylvania in good standing.

  • CPR certification preferred.

  • Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.

  • Experience with Net Health documentation software preferred.

  • Experience with PointClickCare (PCC) preferred.

  • Strong clinical, communication, organization, and interpersonal skills.

  • Ability to work effectively as part of an interdisciplinary team.
