Job description

  • Physician opportunity providing medical care in long-term care settings (SNF, with potential LTAC if desired)

  • Cover one facility per daywith existing advanced practice providers at each site

  • Physician will supervise midlevels, ensure regulatory compliance, and see new admissions within 72 hours

  • Round and go model: choose one facility to visit each day, with additional visits only for urgent patient needs or complex family meetings

  • Total census across all facilities averages about 320 patients (shared with midlevels)

  • Midlevels handle routine phone calls and texts; physician is contacted only for true emergencies

  • Schedule is highly flexible; physician designs their own weekday rounding schedule

  • Engagement can be structured as 1099 independent contractor or W?2 employee (1099 preferred)

  • Compensation is fee-for-service; detailed fee schedule to be shared directly with candidates

  • EMRs used: PointClickCare and ChartPath
