As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

  • Assess patients to identify psychosocial, financial, environmental, and safety needs.

  • Complete initial social work evaluations and reassess patient needs during follow-up visits.

  • Develop and update individualized care plans based on patient needs, goals, and physician orders.

  • Communicate changes in patient condition, concerns, or safety issues to physicians and the care team.

  • Connect patients and families with appropriate community resources and support services.

  • Counsel and educate patients and families to help address social, emotional, and care-related needs.

  • Coordinate with the care team to support continuity of care and effective discharge planning.

  • Provide compassionate, high-quality social work services that support positive patient outcomes.

What is Required?

  • Current State License as a Social Worker

  • Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education

  • 1+ year of social work experience in a healthcare setting

  • Social work skills as defined as general social work standards

  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
