What is a full time concierge nurse practitioner?

A Full Time Concierge Nurse Practitioner is a licensed nurse practitioner who provides personalized healthcare services to patients, often outside of traditional clinical settings, through a membership or retainer model. This role involves comprehensive care, including preventive services, chronic disease management, and wellness consultations. Concierge Nurse Practitioners typically offer greater accessibility, longer appointment times, and enhanced communication with patients. By focusing on individualized care, they help improve patient outcomes and satisfaction. This position usually requires a full-time commitment and may involve home visits, telehealth, and coordination with other healthcare professionals.

How does the role of a full time concierge nurse practitioner differ from traditional clinical nursing positions in terms of daily responsibilities and patient interaction?

As a full-time Concierge Nurse Practitioner, you'll typically manage a smaller panel of patients, allowing for more personalized and in-depth care. Your daily responsibilities often include conducting comprehensive wellness visits, coordinating specialty referrals, and providing direct access to patients via phone, text, or virtual platforms. Unlike traditional settings, you'll spend more time building long-term relationships with patients, proactively managing their health, and collaborating closely with physicians and other healthcare professionals to deliver a seamless, patient-centered experience. This model often results in more autonomy and flexibility, but also requires strong communication and time-management skills.

What are the key skills and qualifications needed to thrive as a full time concierge nurse practitioner, and why are they important?

To thrive as a Full Time Concierge Nurse Practitioner, you need advanced clinical expertise, a graduate-level nursing degree (MSN or DNP), and active Nurse Practitioner licensure. Familiarity with telehealth platforms, electronic health records, and remote patient monitoring tools is typically required. Exceptional interpersonal communication, discretion, and the ability to provide personalized, patient-centered care are vital soft skills in this role. These qualifications ensure high-quality, individualized healthcare delivery and strong patient-provider relationships in a concierge practice setting.

Job description

NURSE PRACTITIONER — PRIMARY CARE

Medical House Calls | Brentwood, TN | Full-Time | Concierge & In-Home Care

──────────────────────────────────────────────────────────

The kind of primary care you’d want for your own family — delivered to theirs.

At Medical House Calls, you won’t rush through a waiting room full of strangers. You’ll walk into your patients’ homes, sit down at their kitchen tables, and take care of entire families; kids, parents, grandparents, as their one consistent provider.

You’ll know their names, their histories, and their dogs. And you’ll have the time to actually practice.

We’re looking for a Family Nurse Practitioner to manage a panel of primary care families across Middle Tennessee, delivering the kind of unhurried, relationship-driven care that patients tell their friends about.

──────────────────────────────────────────────────────────

★★★★★ Over 1,500 five-star reviews • Named a Top Place to Work in 2025

──────────────────────────────────────────────────────────

WHY THIS ROLE IS DIFFERENT

  • Your patients are yours. You manage an assigned panel of families and stay with them over time — real continuity, not a rotating schedule of strangers.
  • Whole-household care. Wellness visits, chronic condition management, acute needs, minor in-home procedures — all ages, all under one roof.
  • Autonomy with support. You’ll exercise independent clinical judgment in the field, backed by collaborative physician oversight and a care team that has your back.
  • A seat at the table as we grow. You’ll help shape how the primary care division expands — and mentor the providers who come after you.
  • No two days look alike. Homes, hotels, offices — you go where your patients are.

WHAT YOU’LL DO

  • Serve as the consistent provider for an assigned panel of primary care families, building continuity of care over time.
  • Deliver longitudinal primary care across the household — preventive and wellness visits, chronic disease management, and acute care as it comes up.
  • Conduct evaluations, physical exams, and comprehensive health assessments.
  • Diagnose, build individualized care plans, and prescribe and administer medications and treatments within your scope.
  • Perform minor in-home procedures consistent with scope of practice and MHC clinical protocols.
  • Order, interpret, and clearly communicate diagnostic and lab results to patients and families.
  • Coordinate care across the broader team, including specialist referrals and follow-up on outside consults.
  • Document encounters accurately and on time in the EHR.
  • Maintain compliance with collaborative practice, supervising physician, and chart review requirements in each state you practice.
  • Participate in on-call or extended-hours coverage as scheduled.
  • Serve as a clinical resource to the MHC team and support onboarding of new providers as we grow.
  • Deliver an experience that keeps families with us — and gets us referred to their neighbors.

WHAT YOU BRING

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) required.
  • Active, unrestricted Nurse Practitioner license in the state(s) of practice required.
  • Family Nurse Practitioner (FNP) certification required.
  • 2–5 years of Nurse Practitioner experience required, in family practice, primary care, or urgent care, with experience managing a continuing patient panel.
  • Current Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
  • Active Drug Enforcement Administration (DEA) registration required, or obtained upon hire where applicable.
  • Valid driver’s license, reliable transportation, and current auto insurance meeting Company requirements.
