Job Title: Social Work Care Manager (MSW)About the Facility
  • Located at 2320 E 93rd St, Chicago

  • 205-bed community hospital

  • Level II NICU

  • Serves a diverse and underserved patient population

  • Strong emphasis on care coordination and community resource management


Position Overview

We are seeking an experienced Social Work Care Manager (MSW) to support care coordination, discharge planning, and psychosocial needs of patients across inpatient units. This role is ideal for a compassionate professional who thrives in a collaborative, patient-centered environment.


Unit Overview
  • Hospital-wide role with assignment to a primary unit

  • Rotation every 2 weeks to support LDRP/NICU (lower caseload during rotation)

  • May float across inpatient units as needed


Patient Population
  • Primarily adult and geriatric patients

  • Underserved populations, including:

    • Unhoused individuals

    • Low-income patients

  • Complex psychosocial and discharge planning needs

  • May support: Med/Surg, PCU, LDRP, and occasionally ICU


Caseload
  • Typically 14-20 patients

  • Reduced caseload when covering LDRP/NICU


Required Qualifications
  • Minimum 3 years of Social Work experience

    • Including at least 2 years in acute care

  • First-time travelers: Accepted

Education & Licensure:

  • MSW (Master of Social Work) - Required

  • Illinois State License (LSW accepted; LCSW not required)


Required Skills
  • Strong experience in:

    • Discharge planning and care coordination

    • Navigating insurance and community resources

  • Ability to:

    • Collaborate effectively with multidisciplinary teams and physicians

    • Conduct thorough psychosocial assessments

    • Identify and coordinate appropriate patient resources

  • Assigned to a primary unit but may float to other inpatient units as needed


Orientation
  • New Employee Orientation (NEO)

  • 3 shifts of department orientation (additional shifts if needed)


Scheduling
  • Shift: Monday-Friday, 8:00 AM - 4:30 PM

    • Occasional late days as needed

  • Weekend Rotation: Every 5th weekend

    • If working weekend, receives following Monday & Friday off

  • On-Call: Not required

  • Holiday Requirement: Approximately 50% (up to 2 holidays per contract)

  • Schedule Type: Set schedule with weekend rotation
