RN Director of Case Management
Northern Georgia

The RN Director of Case Management serves as a subject matter expert in Case Management, demonstrating solid leadership, trust-building, team building, and Change Management abilities to drive departmental success and initiatives. The RN Director of Case Management has an executive presence, excellent communication and presentation skills, proven motivational leadership, and a strong record of delivering results and meeting goals. The RN Director manages daily operations, engages interdisciplinary teams and stakeholders, and is accountable for achieving clinical, service, and financial outcomes.

Qualifications:

  • Registered Nurse (RN) with acute hospital nursing experience required
  • Bachelor Degree in Nursing required
  • Director of Case Management in an Acute Hospital Setting required
  • Masters in Nursing (MSN) preferred
  • Certification in Case Management (CCM/ACM) preferred within 2 years of hire

Responsibilities:

  • Self-starter who manages multiple priorities, work independently on projects, adapt quickly to change
  • Provides strategic leadership and operational oversight for all Case Management, Discharge Planning, Social Work, and Access Control functions
  • Coordinates the development and implementation of standardized workflows and systems to support effective Care Coordination
  • Collaborates with physicians, nursing leadership, and senior administration to optimize patient flow, reduce length of stay, and improve outcomes
  • Ensures timely pre-certification for post-acute services and minimizes organizational financial risk
  • Identifies opportunities for clinical process improvement and works with providers to implement clinical pathways
  • Manages departmental budget, staffing, and performance to maximize productivity and efficiency
  • Oversees data collection, analysis, and reporting of key performance indicators including avoidable days, LOS, and Resource Utilization
  • Participates in interdisciplinary team meetings and Case Management responsibilities
  • Maintains accountability for acute inpatient Social Work functions
