ACTT (Assertive Community Treatment Team)
  • Intensive, multidisciplinary community treatment for individuals with severe and persistent mental health needs
  • High-frequency outreach, crisis response, and long-term stabilization support

CST (Community Support Team)
  • Community-based mental health support focused on recovery, independence, and ongoing engagement
  • Structured support for individuals stepping down from higher intensity services or needing preventative intervention

IIH (Intensive In-Home Services)
  • Family-focused, in-home therapeutic support for children, adolescents, and caregivers
  • Support aimed at reducing risk of out-of-home placement and improving family functioning

Personal Care Services
  • Daily living support (ADLs) for individuals with mental health, disability, or substance use recovery needs
  • Assistance with routines, safety, and maintaining independence

Tailored Care Management
  • Care coordination for individuals with complex behavioral health and social needs
