The Community Health Worker will work primarily within the community at client homes as well as throughout the health care system, and with other agencies/organizations to coordinate care and client needs. Works as a team member within the ConneXions Program.
Responsibilities
Essential Job Functions:
  • Conduct health education and screening outreach in community and healthcare provider settings to provide health education and raise awareness of health concerns and available community resources and services.
  • Reduce client barriers to health care and needed community resources and services. Facilitate communication and coordination between community resources/services and providers.
  • Maintain knowledge and understanding of available community resources, services, and programs. Create connections between vulnerable populations and community services and resources needed.
  • Provide health screenings for conditions and diseases as well as providing health information on those condition/diseases and areas of concern.
  • Follows the scope of work for Traditional/Community Health Workers outlined by OHA. Ability to be cross trained as Birth Doula, Peer Wellness Specialist, and more, as needed.
  • Works closely with Community Health & Outreach personnel, other departments/clinics, and community care coordination teams.
  • The employee supports the hospital mission, vision, values, policies, and procedures.
  • Participates in required education for DNV programs as applicable to position. (reference program education curriculum).
  • Performs other related duties as assigned.

Qualifications
Qualifications:
Education
Required: High School Diploma or equivalent.
Preferred: BA/BS in healthcare field. Training in health education or other healthcare fields.
Licenses/ certifications/ registrations
Required: Valid Oregon driver's license. Certification as a Community Health Worker (CHW) required within 6 months of hire.
Preferred: Certification as a current Community Health Worker (CHW), and/or another relevant healthcare/social services field.
Experience
Required: Prior community outreach and/or medical/office/healthcare experience.
Preferred: Prior experience with teaching or one-on-one counseling for health concerns. In-home visitation experience and phone follow-up experience recommended.
Other:
  • Ability to communicate effectively in oral and written form.
  • Ability to assist and support creative, independent actions for clients.
  • A well-organized self-starter who works well independently, as well as a team member.
  • General computer skills including Microsoft Word, Excel, Outlook, Teams, etc.
  • Proficiency in Spanish bilingual/bicultural preferred.
  • Basic understanding of anatomy/physiology and clinical procedures.

Physical Requirements:
The physical and sensory functions described below are essential to the successful performance of this position. The position requires sufficient auditory ability to detect and respond to various sounds, including verbal communication. Clear and effective communication is essential, as is the ability to interact with patients, families, and healthcare team members. Visual acuity is necessary for reading documentation and monitors, and more. Motor coordination is required for reaching, grasping, handling, and performing fine motor tasks necessary for the use of equipment and procedures. The position also involves occasional stooping, kneeling, or crouching when assisting patients, coworkers or accessing equipment. Walking and standing are typical throughout the shift. The role requires the ability to lift to 50 pounds independently and involves frequent pushing, pulling, carrying, or repositioning of objects weighing up to 25 pounds. May sit for long periods of time.
Working Conditions:
This position is performed primarily in an indoor healthcare environment, such as a hospital, clinic, or long-term care facility. While the work setting is protected from outdoor weather conditions, it may be subject to variable indoor temperatures. The role involves frequent exposure to infectious and contagious diseases, hazardous materials, and medical equipment, with the expectation that all staff follow appropriate safety protocols and utilize personal protective equipment (PPE) as required.
The health care environment can be fast-paced and unpredictable, requiring the ability to respond quickly to changing circumstances, including high-pressure or emergency situations. The work setting also involves multiple simultaneous demands and varying levels of noise due to medical equipment, staff activity, and patient needs.
