Job description

Mountain Community Health Partnership (MCHP) is seeking a full-time Community Health Worker (CHW) Lead. The CHW lead will oversee and support our CHW program while helping individuals and families navigate healthcare and community resources. This role combines direct community engagement, program management, staff supervision, Medicare and health insurance enrollment assistance, and partnership development. The ideal candidate is a strong relationship-builder who is committed to improving health outcomes and reducing barriers to care for underserved populations.
Key Responsibilities:
  • Support onboarding, training, coaching, scheduling, and performance management.
  • Provide daily leadership and supervision of CHW staff.
  • Conduct community outreach and health screenings.
  • Perform Social Determinants of Health (SDOH) screenings and make appropriate referrals.
  • Track program performance measures and grant deliverables.
  • Serve as a Medicare SHIIP Counselor (or obtain certification).
  • Serve as a Certified Application Counselor (CAC) for Health Insurance Marketplace enrollment.
  • Assist with grant reporting, audits, and compliance activities.
  • Track program performance measures and grant deliverables.
  • Represent MCHP at community events, meetings, and health fairs.

Qualifications:
  • Education: High school diploma or equivalent.
  • Experience: Certified Community Health Worker (CHW). Reliable transportation and proof of vehicle insurance. Strong communication, organizational, and interpersonal skills. Experience working in healthcare, public health, social services, or community-based programs preferred. Proficiency with Microsoft Office, including Excel and Word.
