The Community Health Worker (CHW) is responsible for providing health education and outreach to the public across the communities we serve on topics including, but not limited to, obesity, cardiovascular disease, diabetes, cancers, and mental health issues, serving as a bridge between communities, healthcare, and other social systems. CHW's collaborate with physicians, nurses, or health workers to deliver culturally appropriate health education or basic screening services while the providers conduct medical exams or provide medical service, and/or connects people with the services they need to promote disease prevention, exercise, and educational programs to meet the needs of clients. Other duties as assigned.
