Job description

Concordia Visiting Nurses is seeking a Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN). The Nurse Liaison is responsible to coordinate all offered services for home healthcare, this may include completing part or all of the admission process for patients. The Nurse Liaison (RN or LPN) partners with the sales team in developing and maintaining referral source relationships.

This position will be based out of our Middlesex Office location and will travel as needed throughout our Cabot, Baden, and South service area.

Key Responsibilities for the Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN):

  • Provide consultative services to hospitals regarding adult and geriatric patients for homecare and post-acute rehab.
  • Conduct comprehensive assessments to develop homecare and discharge plans.
  • Utilize clinical assessment skills to create tailored care plans for diverse patient needs.
  • Coordinate and document assessments prior to discharge to ensure quality care.
  • Maintain accurate patient documentation using CRM software.
  • Respond to intake inquiries and collaborate with Intake and Clinical Management teams.
  • Build relationships with referral sources and identify new service needs.
  • Educate patients, families, and referral sources on Concordia’s continuum of care.
  • Offer education and counseling on homecare plans and SNF discharge expectations.

Qualifications for the Nurse Liaison (Registered Nurse RN or Licensed Practical Nurse LPN):

  • Current Pennsylvania Licensed Registered Nurse or Licensed Practical Nurse.
  • Minimum of three (3) years of nursing experience required; supervisory and acute care discharge planning experience preferred.
  • Valid PA Driver’s License and auto insurance required.
  • Strong clinical assessment skills are essential.
  • Home healthcare experience is recommended.
  • Ability to work independently while demonstrating sound judgment.
  • Detail-oriented with strong interpersonal skills; respectful and courteous demeanor.
