The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

The Eastman Institute for Oral Health (EIOH) Social Worker is responsible for providing clinical services to patients to assist EIOH professional (and support) staff in the delivery of comprehensive care to EIOH patients. The EIOH social worker is responsible for liaison with community agencies and other relevant providers, which impact patients at EIOH. The EIOH Social Worker provides direct clinical services to identified EIOH patients and their families to increase access to and improve utilization of dental services, on behalf of low-income and vulnerable individuals/families. The EIOH social worker participates in interdisciplinary collaboration with EIOH staff which includes research, training of staff, program development and orientation and training of new staff and residents.

ESSENTIAL FUNCTIONS

  • The EIOH social worker provides psychosocial assessments of referred patients to identify barriers to and adherence with dental care along with identifying and managing risk situation that include child abuse/neglect, domestic violence, concern for harm to oneself or others.
  • The EIOH social worker provides services that address risks and barriers and directly manages the reporting of child abuse/neglect along with coordination of a safety plan with child protective services. The EIOH social worker links patients and families to internal and community resources to provide for basic needs and case management services. The EIOH social worker coordinates care plans with EIOH staff as well as social work colleagues. All social work assessments and services are documented in the dental record according to policy.
  • The social worker participates in interdisciplinary collaboration relevant to program development, QA, new initiatives and teaching. The social worker attends staff meeting and community events relevant to the position.
  • The social worker provides training to dental residents staff and social workers on issues that impact care, disparities in dental disease and access to dental services, child abuse and domestic violence.
  • Other duties as assigned


MINIMUM EDUCATION & EXPERIENCE

  • Master's degree from an accredited school of social work required
  • Or equivalent combination of education and experience


LICENSES AND CERTIFICATIONS

  • LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or
  • If duties are performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required.
