Physical Therapist (PT) – Home Health | Stamford, CT | PRN, Part-Time & Full-Time | Earn Up to $145K

Stern at Home Therapy is hiring Physical Therapists (PTs) to provide one-on-one care to patients in their homes in and around Stamford, CT.

Whether you're looking for a few flexible visits each week, a steady part-time caseload with W-2 benefits, or a full-time career with salary, W-2 per-visit, or 1099 contractor options, we offer multiple ways to build a role that fits your goals.

Full-time Physical Therapists working under our 1099 tiered compensation model have the potential to earn up to $145,000 annually, based on visit volume.

Treat one patient at a time, manage your availability, and provide individualized care without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.

What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Stamford area.
Responsibilities include:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one treatments based on each patient's functional needs and goals.
  • Manage individualized treatment plans with clinical autonomy.
  • Help patients improve mobility, strength, balance, safety, and overall function.
  • Complete accurate and timely clinical documentation.
  • Communicate with patients, families, physicians, and members of the clinical team.
  • Coordinate with our administrative team regarding scheduling, patient care, and logistics.
  • Manage your caseload based on your availability and employment structure.

Compensation:
We offer multiple compensation options based on your level of commitment and preferred employment structure.
PRN:
  • $72 per 4-unit follow-up visit.
  • $85 per evaluation.
Part-Time W-2:
  • $65 per follow-up visit.
  • $75 per 4-unit evaluation.
Full-Time W-2 Tiered Per-Visit Option:
  • $55–$70 per follow-up visit.
  • Earn more per visit as you move through the tiered compensation structure.
  • $75 per 4-unit evaluation.
Full-Time 1099 Tiered Contractor Option:
  • $70–$80 per follow-up visit.
  • Earn more per visit as you move through the tiered compensation structure.
  • $85 per 4-unit evaluation.
  • Earning potential of up to $145,000 annually, based on visit volume.
Full-Time Salary Option:
  • $92,000–$110,000 annually.
