TITLE: Community Health Nurse
Contractual - Start 8/30/26 - End 8/29/27
POSITION: Full-time - 35 hours - Saturday, Sunday, Monday and Tuesday
SALARY: $82,000 - Annually
DEPARTMENT: Family Based Interventions - Family Connects Maryland
LOCATION: 2108 N. Charles Street, Baltimore MD 21218
REPORTS TO: Nurse Manager
SUMMARY: This position provides short term home visiting to all families with newborns delivering at Sinai Hospital. The home visiting nurse will support and assess family needs in 12 domains of physical and psychosocial wellbeing and facilitate referrals and connections to community agencies.
RESPONSIBILITIES:
¨ Follow the Family Connects home visiting protocol. Keep records of client observations and activities as required. Document information for family assessments and program evaluation.
¨ Act as a liaison between Family Connects and Sinai Hospital to ensure that Family Connects scheduling protocol is followed.
¨ Provide health and physical assessment of mother and baby in the home to determine wellbeing.
¨ Assess family needs in the 12 domains of physical and psychosocial wellbeing, including depression, substance abuse and domestic violence. Exercise clinical skills in appraising situations and making decisions.
¨ Establish a trusting relationship with families during home visits to engage families in establishing a family support plan with goals, objectives and activities.
¨ Apply working knowledge of parent-child interaction, maternal-child health, child development and child abuse/neglect to help families improve parenting skills, increase parenting support, reduce stress and address needs of parents.
¨ Maintain 8 open home visit slots and conduct and document at least 6 home visits per week.
¨ Apply effective time management skills, organization and multitasking skills. Documentation is expected to be completed within 24 hours of the visit.
¨ Meet weekly with the Nurse Supervisor to discuss cases and problem solve issues with program implementation.
¨ Completes statistical reports as required.
¨ Attends required staff meetings, training, consultation meetings and conferences.
¨ Participates in agency work teams as assigned.
¨ Other job duties as assigned by the Program Manager or Nurse Supervisor.
EDUCATION:
¨ Associate degree in Nursing from an accredited college or university; Bachelor's Degree preferred
¨ Current and continual licensure as a registered nurse in the State of Maryland required
EXPERIENCE:
¨ 2 years' experience as a registered nurse
¨ Experience working in community-based programs, specifically infant home visiting programs, preferred
¨ Experience and willingness to work with a culturally diverse population
SKILLS:
¨ Knowledge of nursing theory, process, practice and procedures
¨ Knowledge of professional nursing standards
¨ Knowledge of community resources and agencies
¨ Knowledge of Jewish customs, practices, and traditions
¨ Acceptance of individual differences
¨ Knowledge of infant and child development
¨ Ability to build trusting relationships
¨ Excellent oral and written communication skills
¨ Computer skills including Microsoft Office Suite
¨ Ability to speak and write fluent Spanish preferred.
WORK ENVIROMENT:
¨ Must be prepared to travel to various program sites, various communities, and participants' homes.
¨ A driver's license and insured vehicle is necessary to fulfill the duties and responsibilities for the position.
