Job description

Under the supervision of the Case Management Supervisor, the Case Manager is responsible for providing high quality, effective case management to Human Options clients. Case management includes, but is not limited to, comprehensive assessment of needs, development of a care plan, provision of intensive case management services, use of therapeutic interactions to promote health behaviors, coordination of care, and resource linkages to medical, psychiatric, behavioral, educational, and other services as needed. The Case Manager will complete assessments and scales as appropriate and will be responsible for ensuring all documentation is completed in a timely manner.
Duties and Responsibilities
  • Support the Residential Action Plan by meeting program outcomes and grant deliverables.
  • Conduct outreach, enrollment, case management, and documentation in alignment with contract goals and expectations.
  • Assess the financial, social, and emotional needs of the client.
  • Develop individualized care plans to achieve goals designed to prevent decline and improve quality of life by utilizing trauma informed care.
  • Engage with clients in person, in a field-based setting, and on the phone, utilizing evidence-based approaches that promote collaboration between the client and their health, well-being, and safety.
  • Assists with the coordination of medical and behavioral health access issues.
  • Utilize brief behavioral health therapeutic interventions, as necessary, to improve the client's ability to manage their own mental health concerns and symptoms.
  • Assist individuals with the completion of registration paperwork, as needed.
  • Present cases in weekly Case Management Team (CMT) meetings or Multidisciplinary Team (MDT), as needed.
  • Facilitate psychoeducational groups, as needed.

  • Bachelor's Degree required in social work, psychology, counseling, sociology, behavioral science, related field, or commensurate work experience.
  • Three years of experience providing direct services to the target population.
  • Two years of experience in case management with at risk children and families.
  • One year's knowledge of the child welfare system preferred.
  • Familiarity with electronic medical records.
  • Bilingual in English and Spanish preferred.
  • Valid CA Class C license and auto insurance or ability to travel to meetings, trainings, and home visits.
  • Excellent knowledge of community resources and familiarity to utilize resource linkages in Orange County.
  • Knowledge and understanding of issues and challenges of family violence in the community.
  • Proficiency in Microsoft Office, including Outlook, Word, and Excel and working knowledge of computer database program entry and report generation.
  • Complete the 40-hour Domestic Violence Advocate certification training post-hire.
