Nurse Case Manager - CFHP

Position Summary:
Community First Health Plans is seeking a dynamic individual seeking employment in a hospital affiliated health plan. Primarily Monday thru Friday 8:30 a.m. to 5:00 p.m. work schedule. This Professional RN will assist Community First Health Plan (CFHP) members regain optimum health or improved functional capacity by ensuring that members have access to all of the health care services they need in the most efficient and effective manner possible.

Qualifications:

  • BSN preferred
  • Three years acute care experience or managed care experience is required
  • Current RN Texas License
