MDS Coordinator

Job Summary:

The Center for Extended Care at Amherst is seeking a dedicated and compassionate MDS Coordinator to join our team. As a key member of our clinical staff, you will play a vital role in ensuring the highest quality of care for our residents. Our family-owned and operated facility has been dedicated to providing exceptional rehabilitative and skilled nursing services since 1967. Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident

  • Conduct comprehensive assessments, including Minimum Data Set (MDS) assessments, to identify resident needs and develop effective care plans

  • Communicate with residents, families, and healthcare professionals to ensure seamless care coordination and excellent customer service

  • Maintain accurate and up-to-date records and databases to ensure compliance with regulatory requirements

  • Participate in case conferences and care plan reviews to promote continuity of care and improve resident outcomes

  • Leverage expertise and knowledge to support staff development and training, promoting a culture of excellence and innovation

Requirements:

  • MDS Coordinator certification or experience in a similar role

  • Current RN nursing license

  • Strong understanding of MDS assessment and care planning principles
