NPORT Staffing is seeking a licensed Physical Therapist for a full-time, hybrid position in Middletown, NY.

This role combines home care patient evaluations throughout the week with treatment services provided in a skilled nursing facility rehabilitation gym.

The ideal candidate is a motivated clinician who enjoys working across settings, managing a varied caseload, and delivering high-quality, patient-centered care.

Key Responsibilities:

  • Provide skilled physical therapy treatment in a skilled nursing facility rehabilitation gym and home care. 
  • Treat patients with orthopedic, neurological, post-surgical, and geriatric conditions
  • Document evaluations, daily notes, progress reports, and discharge summaries accurately and timely
  • Collaborate with physicians and interdisciplinary healthcare teams
  • Educate patients and caregivers on treatment plans, exercises, and safety
  • Ensure compliance with Medicare guidelines, state regulations, and agency policies.
  • Conduct thorough assessments to evaluate patients' physical and cognitive ability.
  • Individualized Treatment Plans.
  • Develop personalized treatment plans tailored to each patient's unique needs, based on your assessments.
  • Therapeutic Interventions.
  • Administer hands-on physical therapy to patients, both in one-on-one sessions and group settings.
  • Progress Monitoring: Continuously monitor and assess patients' progress, adjusting treatment plans as needed to optimize results.
  • Collaboration: Collaborate closely with a multidisciplinary team, including physicians, and caregivers, to ensure comprehensive patient care.
  • Provide guidance to patients and their families, offering strategies, exercises, and techniques to support ongoing progress and success.

Qualifications:

  • Licensed Physical Therapist in good standing in New York
  • Prior experience in home care and/or skilled nursing facility preferred
  • Graduation from an accredited PT program
  • Passion for patient-centered, in-home care and rehab gym
  • Reliable transportation and willingness to travel locally
  • Strong clinical, communication, and documentation skills
  • Ability to manage multiple evaluations and treatments efficiently
  • Reliable transportation for home care visits
