Job Summary:
Tryfacta is seeking a Home Health Aides for our client in Nassau & Suffolk Counties (Multiple Location). This is a temporary 2 years contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!
Position Title: Home Health Aides
Location: Nassau & Suffolk Counties (Multiple Location)
Duration: 2 years
Work Schedule: Multiples shift flexible with schedule
Responsibilities for this position include, but are not limited to:
  • Provide compassionate care and supervision to individuals with intellectual and developmental disabilities (IDD) in OPWDD residential homes.
  • Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
  • Support individuals in developing independent living skills and achieving person-centered goals.
  • Monitor and report changes in health, behavior, or emotional status to nursing staff or supervisors.
  • Document daily care, activities, and observations accurately.
  • Assist with meal preparation, feeding, and adherence to prescribed diets.
  • Perform light housekeeping, laundry, and maintain a clean and safe living environment.
  • Accompany individuals to medical appointments, community activities, and recreational outings.
  • Follow Individualized Service Plans (ISP) and behavior support plans.
  • Ensure compliance with OPWDD policies, HIPAA, infection control, and safety regulations.
  • Respond appropriately to emergencies and follow established protocols.
  • Work collaboratively with nurses, therapists, supervisors, and interdisciplinary team members.

To be considered for this position, you should have: [Skills, Education, or Experience]
  • Minimum 1 year of Home Health Aide, Direct Support Professional (DSP), CNA, or residential care experience.
  • Experience working with individuals with developmental disabilities, autism, or behavioral health is preferred.
  • Strong communication, teamwork, and documentation skills.
  • New York State Home Health Aide (HHA) Certificate (required)
  • Valid CPR/BLS Certification (American Heart Association or client preferred)
  • Valid First Aid Certification (preferred or required by facility)
  • Valid New York State Driver's License (preferred or required for community outings/transportation)
