Specialty Post Surgical - As a nurse, you can never expect any day to be the same on this unit. We care for a wide variety of surgical patients (ortho, neurosurgery, bariatric, plastics, and colorectal, surgical oncology). We are a fast-paced environment, typically, patients stay for 2-4 days. It is a 32-bed unit, all private rooms.
The Clinical Nurse Leader (CNL) is an expert nurse and role model who facilitates expected outcomes, demonstrates leadership and serves as a resource in the management of clinical nursing care to improve patient outcomes within the defined clinical area of practice and functions to provide holistic, patient-centered care using evidence-based practices. The CNL is responsible for evaluating the appropriateness of admissions and continued patient stay and communicating patient needs to the care management and inter-professional team to facilitate the development of an appropriate care plan. The CNL contributes to the healthcare team's effectiveness by identifying barriers to effective and efficient utilization of resources and evaluating patients' needs. The CNL facilitates communication across the inter-professional team and actively participates in interdisciplinary rounds, complex case review and management of patient throughput. The CNL evaluates, monitors, and improves the quality of nursing care by leading/participating in performance improvement activities and providing/coordinating department-level nursing orientation and ongoing education that support quality patient care and achievement of identified outcome goals. The CNL assists in organizational initiatives as related to ongoing maintenance of specialty designations, regulatory requirements and community collaborative efforts. The CNL provides department-level oversight for the following: Improving clinical outcomes, care coordination Maintain professional practice environment for Nursing Ongoing education and professional development for clinical nurses Manage clinical component of orientation to nursing department Nursing care coordination of complex cases.
Leads improvement initiatives using data to determine need
for change and prioritization of activities. Relies on evidence to determine
the interventions to improve outcomes. Includes colleagues at every level
in improvement projects in the spirit of shared governance. Coordinates
improvement projects to support achievement of organizational goals.
Clinical - Works in collaboration with unit manager, director and CNO to
exceed national mean for nurse-sensitive indicators. Leads and
participates in organizational clinical quality improvement projects and
committees to meet organizational goals and improve patient care.
Achieves annual goas as defined by the organization. Colleague - Works
in collaboration with unit manager, director and CNO to exceed annual
retention goals and defined benchmarks for colleague engagement.
Supports organizational efforts to create a nursing practice environment
that promotes autonomy, transformational leadership, professional
development, collaboration and transition to practice for new graduate
nurses and nurses new to the organization. Financial - Manages
department-based orientation in collaboration with unit manager to
ensure effective and efficient use of educational hours and training.
Achieves target for nursing retention to reduce cost of turnover.
Leads activities to evaluate current practice and stimulate change through the dissemination and implementation of latest evidence-based practice. Contributes to the development of new and revised policies and protocols to improve practice. Relies on the American Nurses Association Standards and Scope of Practice to guide changes in nursing practice and to optimize professional practice. Leverages shared governance to ensure that nurses, techs and other disciplines have input into decisions that impact their practice. Acts as a role model and expert clinical resource in the clinical setting to guide care on a case-by-case basis. Promotes collaboration across disciplines and departments to enhance patient care and collegiality. Holds self and others accountable to living the core values
Leverages clinical ladder program to advance professional development of clinical nurses and achievement of defined department outcomes. Mentors nurses through the clinical ladder process to enhance engagement. Assesses department learning needs and develops educational programs to support clinical growth, professionalism and engagement. Works with individual nurses to help create career path/plan for growth toward long-term career goals. Participates in national nursing specialty organization(s) and contributes to the dissemination of evidence-based practices in area of specialty
Collaborating with the department manager and the education department develops the content and process for department-based orientation. Uses clinical expertise to determine key clinical components of orientation that will ensure safe practice. Tracks progress of orientees and communicates progress to unit manager. In partnership with unit manager, meets with new employees minimally every 2 weeks throughout orientation to communicate progress and address concerns.
Participate in daily multidisciplinary rounds in assigned work area and communication patient needs from the perspective of the discipline of nursing. Communicate plan of care to individual nurses and the broader nursing care team to facilitate patient discharge and successful transition to the next level of care. Ensure implementation of the plan of care by the nursing team. Communicate the plan of care to patients and families, solicit their input to make a plan of care that is meaningful and mutually acceptable to patients and families. Closely collaborate with providers and case management to improve the quality and efficiency of care. Monitor length of stay and escalate care concerns. Participate in complex case review and coordinate care conferences as needed
