The Clinical Integration Specialist (CIS) impacts the overall enterprise-wide clinical integration strategy, focusing on care coordination and collaboration, enhancing effective communication, and developing and implementing standardized protocols and guidelines.   • Impacts the reduction in Home Health hospitalization rates through proactive care coordination and referrals
  • Utilize analytic data (Acclivity and Bamboo) and provide education to internal and external customers and stakeholders on the value of Hospice and Palliative Care.
  • Serve on the Clinical Integration Committee and participate in the committee's effort to advance the strategic goals.
  • Increase internal Home Health referrals to Hospice and Palliative Care by 10% month-over-month.
  • Increase cross-departmental utilization of Palliative Care by 10% quarterly, with a baseline of 2.43% overall across all clinical units.
  • Meet the referral to admission conversion of 85%
Do you have....
  • Graduate of an accredited Practical Nurse Degree, Associate Degree, BSN in Nursing or MSW
  • Current RN or LPN License, or a Licensed MSW
  • 2-5 years of Hospice and Palliative Care experience. Prefer 1 year of clinical experience
  • Must have reliable transportation, a GA valid driver's license, and current vehicle insurance
