Job description

The Center for Human Development, (CHD) is seeking a Case Manager to join our team at Grace House, a residential treatment program for mothers in early recovery. This large historical residential home is located in Northampton, MA. The program can house up to twelve mothers with children in a collective living environment, where they share space and resources in an effort to build a recovery community and encourage peaceful living.

Your role as a Case Manager:

The Case Manager provides support services to women with children and/or pregnant in a comprehensive six-to-nine –month residential treatment program.  S/he assists referred individuals in need of substance abuse treatment by providing the education and support needed to establish a clean and sober way of independent living and assisting clients to access community resources as well as mitigate barriers to success. As a therapeutic setting, we strive to provide each family with a safe, nurturing environment where mothers and their children can experience success. We are looking for someone to work a 11:00am-7:00pm schedule Tuesday-Saturday.

Responsibilities include: 

  • Provide service coordination and linkages to the community
  • Develops helping relationships through use of programmatic structure and evidence based models including Motivational Interviewing (MI)
  • Connects clients to varying and multiple community entities and helps the find resources in order to have a successful treatment and discharge
  • Facilitating groups for up to 12 women at a time
  • Assist clients in obtaining benefits, housing and health care as needed
  • Provide support and participate in the development, revision and utilization of relapse prevention and/or safety plans
  • Encourages and facilitates utilization of clients’ natural support systems as well as recovery-oriented, peer support and self-help supports and services
  • Will assist in data collection and tracking for grant evaluation
  • Maintain and update appropriate treatment plans for residents

 REQUIRMENTS:

  • Bachelor’s degree from an accredited educational institution or at least three years of relevant work or lived experience  that is acceptable by MBHP waiver
  • Ability to pass certification for CPR, First Aid
  • Valid driver’s license and ability to provide transportation on an as needed basis
  • Ability to counsel and support clients with co-occurring and/or substance use disorder diagnoses
  • Demonstrated knowledge of trauma-informed care
  • Candidates that are Bilingual in Spanish/English are encouraged to apply
