JOB PURPOSE:ย The Home Study and Post-Release Services (HSPRS) Case Managerย Level 2ย willย provideย case management and supportive services to unaccompanied immigrant children released from federal custody.ย The HSPRS Case Manager will work in collaboration with program staff and supervisors to cultivate a client-centered and trauma-informed approach to service provisions.ย
ESSENTIALย JOBย RESPONSIBILITIES:ย
- Conduct home visits and phone contacts with the dual purpose of assessing safety andย appropriateness ofย childโsย placement with their caregiver (sponsor) and connecting childย to services in the community to address risk factors.ย
- Ongoing case management with a focus on screening and makingย appropriate referralsย toย address childโs needs in the following domains: Placement issues, safety issues,ย education, health, mental health, legal/immigration proceedings, substance abuse,ย independent living, criminal/juvenile justice, child enrichment activities.ย
- Conduct age-appropriate comprehensive assessment in aย child centered, trauma-informed manner and documented inย an online case management system.ย
- Provide crisis intervention services directly, as needed, or connect family to services toย ensureย minorโsย safety and well-being inย a crisis situation.ย
- Activelyย establish,ย enhanceย andย maintainย liaison relationships with key governmentย agencies, and non-profits/ community organizations.ย
- Comply withย policies and procedures governing the program and ensure that the programย isย operatingย at maximum efficiency.ย
- Meet or exceed program productivity expectations on a regular and sustained basis.ย
- Other duties as assigned.

ESSENTIALย QUALIFICATIONS:ย ย
- Minimum of 1 years of experience relevant to the position and working with vulnerable or migrant populations; 3 years preferred.ย
- Proficiencyย in Spanish/English requiredย (written and spoken language skills).
- Excellent verbal and written communication skills.
- Ability to handle confidential material in a responsible manner.ย
- Able and willing to travel domestically up to 50% ofย time.ย
- Proficiencyย in Microsoft Office applications.
EDUCATION:ย Bachelorโs degree in the education, psychology, sociology, or other behavioral sciences equivalent.ย
EXPERIENCE:ย ย Seeย Essential Qualifications and Education.ย
ATTENDANCE:ย Mustย maintainย regular and acceptable attendance at such level as isย determinedย in the employerโs sole discretion.ย
LICENSES:ย Driverโsย License withย clear record.ย ย
VEHICLE:ย Must have daily use of a vehicle without prior notice.ย
