Job description

Coleman Health Services, a nationally recognized not-for-profit mental health and substance use services provider is seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Marion and Crawford counties. The Case Manager will assist with accessing community-based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have BA in related field, one-year related experience and knowledge of community resources. This position will be based out of our office in Marion, Ohio.

Job Duties of Case Manager includes, but not limited, to:

  • Assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services.
  • Transition clients and families into ongoing care
  • Work collaboratively with treatment teams internally and externally to ensure safe transitions

Qualifications of the Case Manager

  • Bachelor's degree in a social service-related field or three-years' experience
  • Willing to get NPI and Medicaid Billing number (we'll help!)
  • Maintain excellent driving record
