About Us

Northside Youth And Senior Service Center (NSYSSC) is a trusted nonprofit committed to supporting the health, independence, and well-being of older adults in North St. Louis. We are currently seeking a compassionate and self-motivated Senior Services Case Manager to join our team as an independent contractor (1099). This is a flexible position ideal for professionals who are passionate about working with seniors and want to manage their own schedule.

Position:

Senior Services Case Manager (Contractual - 1099)

Key Responsibilities:

  • Conduct comprehensive assessments of senior clients to develop personalized service plans
  • Provide counseling, resources, and support to seniors and their families
  • Facilitate and coordinate senior-focused programs, activities, and referrals
  • Perform benefit assessments and periodic reassessments
  • Maintain accurate and confidential client records and documentation
  • Collaborate with internal team members and external partners to enhance client care

Requirements:

  • Bachelors degree in social services, gerontology, or a related field
