Join the excitement of a dynamic company committed to providing exceptional neurological rehabilitation to adults with brain injury! Learning Services, a Collage Rehabilitation Partner, is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. For more than 40 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

We are searching for a Case Manager for our program located in Raleigh, NC.


Essential duties and functions

  • Responsible for all case provisions for the active rehabilitation caseload, for both the residential neurorehabilitation and day treatment clients
  • Leads team meetings and interfaces with the treatment team to establish rehabilitation goals and coordinates treatment necessary to support the client’s treatment plan
  • Responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up
  • In consultation with the clinical team, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team
  • Serves as client advocate by communicating clients’ needs and issues to team members and other designated individuals, including a plan for resolution and follow-through as indicated
  • Support the Program Director in program case managers’, onboard training, and performance to ensure case management standards of performance meet expectations

Requirements

  • State licensure as a Registered Nurse is preferred
  • A bachelor's degree along with case management and health care industry experience is required
  • A master's degree and CRC or CCM designation is preferred
  • Must have a valid driver's license and maintain responsible driving behaviors
