Assistant Director of Nursing (ADON) / RN Case Manager
Position Summary
The Assistant Director of Nursing (ADON) / RN Case Manager supports the Director of Nursing in overseeing clinical operations, ensuring quality patient care, and maintaining compliance with agency policies and state regulations. This role supervises field staff, conducts client assessments, develops care plans, and coordinates home health services to promote positive patient outcomes.
Key Responsibilities
  • Conduct client assessments, admissions, and reassessments.
  • Develop and manage individualized Plans of Care.
  • Supervise and support nurses, caregivers, and home health aides.
  • Provide clinical guidance, training, and competency evaluations.
  • Monitor documentation for accuracy and regulatory compliance.
  • Coordinate care with physicians, families, and community resources.
  • Assist with quality assurance, audits, and accreditation readiness.
  • Support the Director of Nursing with daily clinical operations and staff management.
  • Participate in on-call rotation as needed.
Qualifications
  • Current Indiana RN license in good standing.
  • Minimum 2 years of nursing experience; home health experience preferred.
  • Previous supervisory or case management experience preferred.
  • Strong clinical assessment, communication, and organizational skills.
  • Valid driver's license and reliable transportation.
