Job description

Position: Case Manager 

Status: Full-Time

Responsibilities: 

  • Conduct intake orientation with all patients, explaining rules, policies and procedures
  • Complete Case Management assessments and reports
  • Develop working relationships with outside agencies
  • Evaluate patient needs and providing referrals as appropriate.
  • Liaison between referral sources and provide updates on patient progress in treatment
  • Assist patient in obtaining necessary resources (housing, food stamps, medical and benefit assistance)
