POSITION SUMMARY
The primary function of the Care Coordinator is to provide care coordination, in accordance with CCBHC certification criteria, for persons served and their families. The Care Coordinator is responsible for outpatient primary care screening and monitoring, gathering vital data to determine health care needs, providing support and connecting individuals with community resources throughout the course of treatment. This position also serves as a member of the individual's care team, collaborating with a variety of providers throughout the community. The Care Coordinator is committed to providing high quality service and promoting easy access to care, including transitions, at all levels. This also includes administration and collection of data, using the National Outcomes Measures (NOMs) tool, as required. This position requires an individual to be adaptive, organized, self-motivated and professional with a wide range of people in a variety of settings. Care Coordinators are expected to work both independently and collaboratively and demonstrate strong verbal and written communication skills.
ESSENTIAL FUNCTIONS
โ€ข Provision of care coordination services to individuals and families, based on assessed needs.
โ€ข Ongoing collaboration with treatment team members, including individuals, families, direct care providers and community partners throughout the course of treatment.
โ€ข Facilitate collateral communication between persons served, support staff, direct care providers, and the community to achieve desired outcomes.
โ€ข Facilitate and support persons served to ensure successful transitions throughout the course of treatment.
โ€ข Develop and maintain collaborative relationships within agency departments and in the community setting, through participation in activities and events.
โ€ข Administer the NOMs, Social Drivers of Health Screening, Health Screening at admission and other specified intervals.
โ€ข Assessment and documentation of vitals at admission and other specified intervals.
โ€ข Documentation of all assessment data in accordance with program expectations.
โ€ข Collaborate with all treatment team members regularly.
โ€ข Communicate essential information to key staff prior to and after appointments.
โ€ข Ensure persons served have all necessary information to complete next steps in treatment prior to leaving the building after initial appointments.
โ€ข Active and timely participation in all administrative and treatment team meetings for purpose of case review, case assignments, and individual planning.
โ€ข Documentation is timely, current, and accurately reflects service provision and response to those services.
โ€ข Act as an advocate for persons served. This includes identifying gaps and developing proposals for meeting needs and accessing resources and services.
โ€ข Work as part of an integrated treatment team collaborating with the individual served, family members or others they want involved in their treatment and other treatment team members (internal and external) to help support the individual in their treatment and recovery.
โ€ข Demonstrate cultural and linguistic competence.
โ€ข Approach interactions with individuals served and colleagues with a trauma sensitive lens
โ€ข Provide services that are recovery oriented, and person and family centered, utilizing a strengths-based approach.
โ€ข Provide services to meet the whole health needs of individuals served.
โ€ข Utilize appropriate assessment instruments to help monitor the progress of individuals served and ensure necessary services are received in the appropriate intensity and duration.
โ€ข Utilize Evidence Based Practices (EBPs) as appropriate.
โ€ข Participate in internal and external networking activities as designated.
โ€ข Complete required training for position.
โ€ข Respect and demonstrate ability to maintain the confidentiality of persons served.
โ€ข Act as an ambassador of Family Service and Guidance Center in the community setting.
โ€ข Support and actively promote the vision and mission of Family Service and Guidance Center and the philosophy of the strengths perspective.
โ€ข Other duties as necessary or required for the fulfillment of the CCBHC program, SAMHSA grant, or other strategic initiatives.

POSITION QUALIFICATIONS
Education/Licensure:
โ€ข Bachelor's Degree in human services field or be equivalently qualified by work experience or a combination of work experience and education.
โ€ข High school diploma required.
โ€ข Must successfully pass the KBI, DCF Central Registry and all background investigations.
โ€ข Must possess a valid state of Kansas Driver's license, a good driving record, current auto insurance and reliable transportation.
Experience:
โ€ข Familiarity with the Community Mental Health System in Kansas and admissions preferred.
โ€ข Experience working with vulnerable/marginalized populations preferred.
โ€ข Experience with interviewing and the intake process preferred.
