Registered Nurse - Nights

The Role

We're hiring a Registered Nurse to monitor patient vitals and alerts, triage inbound clinical calls, and manage patient care after normal business hours. This role sits at the center of Cadence's overnight and weekend care coverage, working alongside Nurse Practitioners and each patient's Physician to manage treatment plans and medication adherence for patients with CHF, hypertension, and diabetes. You'll be the clinical point of contact for patients once their care team's business hours have ended.

Available Schedules:

  • Thursday, Friday, Saturday, and every other Wednesday, 7:00 PM to 7:00 AM ET.
  • Sunday, Monday, Tuesday, and every other Wednesday, 7:00 PM to 7:00 AM ET.
What You'll Do
  • Triage inbound patient calls using standardized clinical protocols, directing patients to the appropriate level of care, from after-hours clinic lines to emergency escalation.
  • Follow up with patients reporting abnormal vitals to assess clinical status and determine the appropriate triage pathway.
  • Monitor patient vitals, symptoms, and labs in real time to identify patients who need clinical intervention or escalation.
  • Manage patient treatment plans and medication adherence in collaboration with Cadence Nurse Practitioners and each patient's Physician.
  • Document patient case notes in the Cadence platform and log follow-up tasks and tickets in Zendesk to keep the care team aligned.
  • Respond to inbound clinical questions and escalations by phone and text, and re-engage patients who have lapsed on vitals or need rescheduling.
  • Deliver a consistent, high-quality care experience that meets the standards of Cadence's health system partners.
What You Need
  • 4+ years of clinical experience treating patients with CHF, hypertension, and diabetes in an inpatient or outpatient setting.
  • Active, unencumbered Registered Nurse license with multi-state compact licensure (RN Compact).
  • Experience working in a CHF bridge clinic or similar chronic disease management setting.
  • Experience working with remote patient monitoring technology and platforms.
  • Demonstrated ability to independently triage abnormal vitals and lead virtual follow-ups supporting program enrollment, treatment plan changes, and medication adherence.
  • Availability to work the assigned overnight and weekend schedule (Sunday, Monday, Tuesday, and every other Wednesday, 7:00 PM to 7:00 AM ET).
  • Fluency with AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) and the ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable.
Compensation

The anticipated compensation range for this role is $80,000 - $90,000 salary, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
