Job description

Join a compassionate healthcare team providing hospice care in the Midwest region of the United States. This role involves conducting face-to-face assessments and delivering healthcare services under the general supervision of medical directors and hospice physicians. You will play a vital role in evaluating patients' physical and psychosocial health through comprehensive assessments, ensuring timely completion within agency guidelines.
Key Responsibilities:

  • Perform thorough health assessments and document findings accurately and promptly.
  • Collaborate closely with physicians, patients, families, and interdisciplinary teams to develop and update individualized care plans.
  • Maintain respectful and positive relationships with patients, families, colleagues, and referral sources.
  • Adhere strictly to privacy and security regulations concerning Protected Health Information (PHI).
Education and Experience:
  • Master of Science in Nursing (MSN) or advanced practice degree required.
  • Certification in Family Practice or Certified Hospice and Palliative Nurse (CHPN) within three months of hire.
  • Preferred: Experience in hospice and/or oncology care.
Licensure and Certifications:
  • Current APRN licensure in the relevant state.
  • National Provider Identifier (NPI) and DEA numbers.
  • Basic Life Support (BLS) certification required within three months of hire.
  • Valid driver's license and auto insurance meeting state requirements.
