Hospice Registered Nurse (RN) - Case Manager

Location: Darke County, OH Territory
Job Type: Full-Time | Permanent | Direct Hire
Schedule: Monday-Friday | 8:00 AM-4:30 PM | Every 4th Weekend Rotation

Compensation
Starting at $36.50/hr (ADN) | $37.90/hr (BSN)
Pay increases based on experience

Additional Earnings:
- CHPN Certification: +$0.25/hr
- Mileage Reimbursement: $0.526/mile
- $200 annual clothing allowance
- $40/month cell phone reimbursement

Join a growing hospice organization providing compassionate, patient-centered care throughout the Darke County territory. Case managers typically complete 4-5 patient visits per day while managing a caseload of 20-30 patients alongside an LPN. Hospice or home health experience is preferred, but nurses with acute care or LTC/SNF backgrounds are encouraged to apply and will be considered for training.

Responsibilities:
- Manage hospice patient caseloads and coordinate care plans
- Complete patient assessments and routine visits
- Collaborate with the interdisciplinary hospice team
- Document care using Netsmart EMR
- Educate and support patients and families

Requirements:
- Active RN license
- Hospice or Home Health experience preferred
- Acute care or LTC/SNF experience considered
- Strong communication, flexibility, and compassionate patient care
