Job description

Hospice House/Inpatient Unit Registered Nurse

As an RN in our Hospice House/Inpatient Unit, you'll provide compassionate, patient-centered care to individuals with life-limiting illnesses while supporting their families through every stage of the hospice journey. Working alongside an interdisciplinary team, you'll deliver high-quality clinical care in a collaborative and supportive environment.

Responsibilities include:

  • Deliver skilled nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
  • Assess patients' physical, emotional, psychosocial, and spiritual needs and incorporate findings into individualized care plans.
  • Develop, implement, and regularly update patient-centered plans of care in collaboration with patients, families, and the care team.
  • Monitor changes in patient condition and communicate updates to the physician to ensure appropriate levels of care.
  • Review and adjust interventions, medications, and visit frequency as patient needs evolve.
  • Participate in interdisciplinary team meetings, care conferences, and admissions to coordinate comprehensive patient care.
  • Accurately document assessments, interventions, physician orders, and patient status in the electronic medical record.
  • Communicate patient updates through effective handoff methods, including SBAR, to support continuity of care.
  • Provide guidance and oversight to personal care specialists, volunteers, and other caregivers to ensure care aligns with the established plan.
  • Assist other departments and team members as needed to support exceptional patient care.
  • Participate in quality improvement initiatives that enhance patient outcomes and organizational excellence.
  • Serve as a preceptor for new team members when assigned.

Qualifications include:

  • Current Registered Nurse (RN) license in good standing with no disciplinary actions.
  • Current CPR certification.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred. Support is available for eligible employees pursuing certification.
  • Strong clinical assessment, symptom management, and care planning skills.
  • Experience documenting patient care in an electronic medical record system.
  • Valid Ohio driver's license, reliable transportation, and proof of auto insurance.
  • Ability to successfully complete a criminal background check and 10-panel drug screening.
