Work Environment

The position is based onsite at our state-of-the-art rehabilitation center located in Tracy, California. Our center is equipped with cutting-edge facilities and advanced therapeutic tools, creating an ideal environment for both patients and staff. The daily operations run Monday to Friday, from 9 AM to 5 PM, allowing for a balanced work-life routine. Key Responsibilities
  • Conduct thorough initial assessments of patients' medical histories, functional limitations, and personal health goals to develop customized care plans.
  • Formulate individualized treatment strategies based on assessment findings, ensuring each patient receives tailored care to meet their unique needs.
  • Educate patients on their treatment journey, effectively communicating the details of their care plan to foster agreement and commitment.
  • Utilize a variety of treatment techniques such as progressive exercise prescription, manual therapy methods, and patient education to enhance recovery.
  • Regularly monitor patients' progress and adjust plans of care as necessary, ensuring that treatment outcomes align with patient goals.
  • Maintain comprehensive documentation of services rendered, including clinical progress notes and treatment modifications, adhering to best practices in patient record management.
Patient Population / Client Focus

The primary patient population served by our rehabilitation center consists of individuals recovering from orthopedic injuries, surgical rehabilitations, and chronic pain conditions. Qualifications
  • A Doctoral Degree in Physical Therapy (DPT) from an accredited institution.
