Choice Health at Home is seeking an experienced Home Health Business Development Liaison to drive referral growth and market expansion throughout West Broward County. The ideal candidate has a proven track record of generating home health referrals, developing relationships with physicians and facilities, and consistently achieving admissions goals.

What You'll Do
  • Develop and maintain strategic relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and case managers
  • Grow market share by identifying and securing new referral sources
  • Execute territory development plans and achieve monthly admission targets
  • Serve as a trusted resource and clinical liaison for referral partners
  • Analyze referral trends and implement growth strategies
  • Collaborate with clinical leadership to ensure exceptional patient outcomes and customer satisfaction
Required Experience
  • Minimum 2-3 years of successful home health or hospice sales experience
  • Stable work history in the healthcare sales field
  • Demonstrated history of meeting or exceeding referral and admissions goals
  • Established healthcare referral relationships within Broward County strongly preferred
  • Strong knowledge of Medicare, home health regulations, and post-acute care services
  • Experience using CRM systems and managing a sales territory
  • HomeCare HomeBase experience preferred
