Job description

Spine and Rehabilitation Medicine Associates is seeking a skilled and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to provide physiatry consultative services within a nursing home setting. This part-time role focuses on delivering high-quality, patient-centered rehabilitative care, including both new patient evaluations and ongoing follow-up visits.
Key Responsibilities:
  • Conduct comprehensive new patient evaluations and follow-up visits for residents requiring physiatry care
  • Review and interpret medical records, therapy notes, and interdisciplinary documentation
  • Develop and implement individualized treatment plans focused on functional improvement and pain management
  • Perform procedures and injections as clinically appropriate
  • Collaborate with interdisciplinary team members, including physicians, therapists, and nursing staff
  • Participate in care planning meetings and team conferences
  • Maintain accurate, timely, and compliant documentation
  • Manage a daily patient volume of approximately 20 patients
Qualifications:
  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
  • Experience in physiatry, rehabilitation medicine, orthopedics, or pain management preferred
  • Prior experience working in a nursing home or skilled nursing facility strongly preferred
  • Proficiency in performing musculoskeletal injections and related procedures
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and teamwork abilities
