American Physiatry is seeking a Nurse Practitioner to join our team serving residents in skilled nursing facilities across Maryland. This is a unique opportunity for an NP who enjoys working independently, building relationships, and improving the care of medically complex older adults.
This field-based role involves traveling to skilled nursing facilities throughout the state to meet with residents who have multiple chronic medical conditions. You will work closely with patients, families, and facility staff to provide education, support care planning, and help ensure patients receive the resources and follow-up they need to achieve the best possible outcomes.
The ideal candidate is self-motivated, organized, comfortable working autonomously, and passionate about caring for the geriatric population.
Responsibilities:
Travel to skilled nursing facilities throughout Maryland to meet with residents and conduct patient visits.
Identify residents who may benefit from additional care coordination and ongoing support.
Educate patients and families regarding chronic conditions, medications, and treatment recommendations.
Develop and maintain individualized care plans for medically complex patients.
Collaborate with facility staff, primary care providers, specialists, and other members of the healthcare team.
Support residents during transitions of care and help reduce avoidable hospitalizations and emergency department visits.
Complete accurate and timely documentation in the electronic medical record.
Build and maintain strong relationships with facility leadership and interdisciplinary teams.
Qualifications:
Active Maryland Nurse Practitioner license.
Board certification as an FNP, AGNP, Adult-Gerontology Primary Care NP, or Adult-Gerontology Acute Care NP.
Experience in skilled nursing, long-term care, geriatrics, primary care, home health, hospice, or chronic disease management preferred.
Excellent communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Strong organizational and documentation skills.
Valid driver's license and reliable transportation.
Willingness to travel extensively throughout Maryland. Occasional overnight travel may be required depending on facility locations.
