Job Summary
Under the direct supervision of the Adult Protective Supervisor/Assistant Supervisor, the APS Case Worker receives and responds to intakes, referrals, and crisis situations as assigned. The primary responsibilities include assessing reports of abuse, neglect, or exploitation, conducting casework, following up on substantiated cases, and completing documentation in accordance with Illinois Department on Aging (IDoA) guidelines.
This position carries out case managing responsibilities in accordance with organizational policies and applicable laws.
Essential Duties and Responsibilities
  • Conduct home visits to assess clients' needs and assist clients in accessing services.
  • Complete an investigation assessment for adult abuse & exploitation, collect concrete evidence, and interview alleged victims, abusers and collaterals.
  • Determine the need for emergency intervention services, provide crisis intervention and participate in 24/7 on call responses for the cases.
  • Seek & coordinate assistance from other providers on behalf of clients.
  • Meet all required response timeframes for investigations, case work, and follow up, at risk assessments, and case closures that are established by State Law and IDOA Policies and Procedures.
  • Consult with supervisor to determine whether a case should be substantiated, unsubstantiated, or unable to be substantiated.
  • Develop a case plan in consult with the client and supervisor
  • Provide services as needed, including OP's, guardianships and other legal interventions, emergency safe shelter, after-hour services and EIS.
  • Accurately complete and process referrals for clients to other agencies and private individuals.
  • Accurate completion of all documents, submit files to IDOA as required. Complete VOA reports
  • Provide monitoring, follow-up and assessment for ongoing risk for each client on assigned caseload
  • Prepare and present cases for M-TEAM meetings and incorporate recommendations into client case plans.
  • Work with staff, other agencies and private individuals to find hard-to-reach clients.
  • Maintain timely and accurate required documentation of service given to clients.
  • Work closely with other casework and case management staff.
  • Participation in activities and events to promote agency services, may include giving presentations at various locations on benefits and services, is required.
