• Must have 3+ years of leadership experience overseeing physicians• Leadership position with a 50% leadership / 50% clinical split• Work schedule: Monday–Friday, 7:30a–5p• Full-time position• Value-based care model• See patients on a monthly basis• Small patient panels, typically 350–450 patients per physician• On-call responsibilities limited to your own patient panel• In most situations, physicians are not required to supervise APPs• Clinical population consists primarily of Medicare Advantage patients age 65 and older• Average patient age is approximately 71–72, often with multiple chronic conditions• Focus on geriatric primary care and complex chronic disease management• The practice does not typically see children or younger adults• EMR: proprietary platform• No 2026 or 2027 residents• No visa sponsorship available
