Rivertown Health and Rehabilitation Center is seeking a highly organized, compassionate, and detail-oriented Central Intake Coordinator to join our growing skilled nursing team in Montgomery, Alabama. This position plays a critical role in managing referrals, coordinating admissions, and serving as a key communication link between hospitals, providers, families, and our interdisciplinary care team.
Job Requirements:
  • Previous experience in healthcare admissions, intake coordination, case management, or skilled nursing preferred
  • Knowledge of Medicare, Medicaid, managed care, and insurance authorization processes preferred
  • Strong communication and interpersonal skills
  • Excellent organizational and multitasking abilities
  • Ability to work efficiently in a fast-paced environment
  • Proficiency with electronic medical records and Microsoft Office
  • High school diploma or equivalent required; healthcare-related education preferred

Job Responsibilities:
  • Manage and coordinate incoming referrals and admissions for the skilled nursing facility
