Position Summary

We are seeking a compassionate, organized, and dedicated Social Services Designee to join our skilled nursing team. The Social Services Assistant works closely with the Social Services Director to support residents and their families by assisting with admissions, discharge planning, care coordination, resident advocacy, and ensuring residents’ psychosocial needs are met. This position plays an essential role in promoting each resident’s quality of life while maintaining compliance with federal, state, and facility regulations.

Essential Duties and Responsibilities

  • Assist the Social Services Director with resident assessments and care planning.
  • Meet with residents and families to provide support, education, and community resources.
  • Assist with admission, transfer, and discharge planning.
  • Help coordinate appointments, transportation, and outside services.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Participate in interdisciplinary care plan meetings.
  • Assist residents in obtaining community resources and benefits when appropriate.
  • Address resident concerns and report grievances according to facility policy.
  • Support resident rights and advocate for resident well-being.
  • Assist with advance directives and related documentation.
  • Coordinate resident and family meetings as needed.
  • Maintain confidentiality in accordance with HIPAA regulations.
  • Perform other duties as assigned by the Social Services Director or Administrator.

Qualifications

Required:

  • High school diploma or GED
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management abilities.
  • Proficiency with Microsoft Office and electronic medical records.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary team members.

Preferred:

  • Experience in a skilled nursing facility, long-term care, rehabilitation, healthcare, or social services.

  • Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
