What is an MLTC RN?

MLTC RNs, or Managed Long Term Care Registered Nurses, are healthcare professionals who assess and coordinate long-term care services for patients enrolled in Managed Long Term Care (MLTC) plans. They evaluate patients’ medical, functional, and psychosocial needs, develop care plans, and help ensure that members receive appropriate home and community-based services. MLTC RNs often work with elderly or disabled individuals to help them remain safely in their homes while managing chronic conditions.
