Occupational Therapist OT Home Health (PRN)

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in west Pasco county (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in west Pasco county (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas).

Schedule: PRN; 5-10+ home health visits per week.

What You'll Do:

  • Perform comprehensive patient assessments, including OASIS assessments at required intervals
  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled occupational therapy services and treatments as outlined in the plan of care
  • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need:

  • Active Florida OT license
  • Minimum of 1 year experience as a licensed Occupational Therapist (OT)
  • Home health care experience strongly preferred
  • Prefers experience using Homecare Homebase (HCHB) EMR
  • Strong assessment, critical thinking, and patient education skills
