Job Title: Homecare Homebase workflow Specialist - Hospice
Office: University of Tennessee Hospice
Status: Part Time
POSITION SUMMARY: The HCHB Workflow Specialist will perform all necessary hospice workflow clinical tasks for multi-site locations to ensure the timely review, edit and flow of tasks within HCHB for all disciplines on the hospice team.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:
  • Reviews hospice Start of Care Evaluations.
  • Reviews and verifies admission entry and creates visits with frequencies.
  • Reviews and verifies all initial Plans of Care to ensure adherence to written specifications.
  • Reviews, edits, and locks all Hospice Item Sets (HIS) upon admission and discharge.
  • Creates all Certificates of Terminal Illness (CTI).
  • Reviews, edits, and approves all Recertifications.
  • Reviews and approves all physician's orders.
  • Monitors and verifies all visit coordination notes.
  • Ensures Recertification and Discharge decision workflow is processed timely.
  • Utilizes current technology to fulfill job functions.
  • Other appropriate services and special projects.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:
  • Current RN licensure in state of practice.
  • Graduate of accredited school of nursing with a BSN degree is preferred.
  • At least one (1) year of experience working as a Registered Nurse in a hospice setting.
  • Management or supervisory experience and nursing experience in a hospice setting preferred.
  • Understanding of Medicare regulations related to hospice care.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

PREFERRED KNOWLEDGE AND SKILLS:
  • Excellent organization, time management, and problem-solving skills.
  • Must have outstanding verbal and written communication skills.
  • Strong computer skills required with prior HCHB experience preferred.

SUPERVISORY RESPONSIBILITIES:
  • None
