A Day in the Life of a Hospice Social Worker (Weekend)
As a Hospice Social Worker, your weekend shifts are focused on connection and compassion. You'll meet with patients and families, provide emotional support, and guide them through the hospice journey. This Social Worker role includes collaborating with nurses and the care team, advocating for patient needs, and ensuring comfort during critical weekend moments. Every Hospice Social Worker brings calm, clarity, and care when it matters most.
What You'll Do (Hospice Social Worker Responsibilities)
- Complete psychosocial assessments in hospice careÂ
- Develop care plans and provide counseling as a Social WorkerÂ
- Coordinate community resources and referralsÂ
- Support advance care planning (DPOA-HC)Â
- Maintain timely, accurate documentationÂ
- Collaborate with the interdisciplinary hospice teamÂ
- Ensure compliance with Medicare and Illinois standardsÂ
Schedule (Weekend Role)
- Saturday & SundayÂ
- 12-hour shifts (8:00am - 8:30pm)
- Consistent weekend schedule for work-life balanceÂ
What You Bring (Requirements)
- Master's Degree in Social Work (MSW)Â
- Illinois LSW or LCSW preferredÂ
- 1+ year experience as a Social Worker (healthcare or hospice preferred)Â
- Strong communication, empathy, and organizational skillsÂ
- Valid driver's license and reliable transportationÂ
- Â Local residence with ability to respond within 30 minutes of assigned service area. Meets agency requirements for criminal background check.Â
Pay & Benefits

- Wage: $31.88 - $44.63 per hour (based on experience)
