Travel RN Weekend Shifts in Decatur, IL

Provide nursing care during Friday, Saturday, and Sunday shifts in a skilled nursing/rehabilitation facility. Deliver quality care to geriatric patients while benefiting from competitive compensation and comprehensive benefits.

Key Responsibilities:
  • Provide comprehensive nursing care to patients in a SNF/Rehab and Long Term Care setting during scheduled weekend shifts (Friday/Saturday/Sunday).
  • Execute medication administration, treatment coordination, and monitoring according to physician orders and facility protocols.
  • Collaborate with interdisciplinary teams (rehab therapy, nursing assistants, and facility leadership) to support patient outcomes and care plan goals.
  • Maintain accurate, timely documentation including nursing notes, vitals, wound/skin assessments, and response to interventions.
  • Document and communicate patient condition changes promptly to the charge nurse and/or provider, ensuring continuity of care throughout the shift.
Qualifications:
  • Graduate of an accredited nursing program with an RN degree (ADN or BSN).
  • Current CPR certification (BLS).
  • Active RN license in the state of practice (Illinois) or compact/multistate eligibility if applicable.
  • Minimum 1 year of recent experience in SNF/Rehab or Long Term Care settings.
  • Experience caring for geriatric patients with complex chronic conditions and rehabilitation needs.
  • Demonstrated competency with wound care, medication administration, and care plan documentation.
