Weekend Visit Nurse (LPN)
Every other weekend - Saturday & Sunday
8-10 visits per day - $27.50 - 38.50 per visit
We are seeking an experienced Visit Nurse (LPN) to support and oversee high-quality in-home patient care. This role is responsible for clinical oversight, quality assurance, patient and caregiver satisfaction, and effective coordination of care across multiple locations. This role is every other weekend (Saturday & Sunday), must meet visit requirements per day.

Key Responsibilities
  • Utilize effective problem-solving and clinical decision-making skills to identify issues, determine root causes, develop solutions, and evaluate outcomes
  • Conduct client visits and participate in quality improvement initiatives to ensure service satisfaction
  • Investigate and resolve assigned patient and staff complaints in a timely and professional manner
  • Ensure all clinical documentation is accurate, complete, timely, and compliant with company policies, procedures, and regulatory requirements
  • Assess patient status and interpret clinical data to identify age-specific and diagnosis-related care needs
  • Coordinate and communicate effectively with patients, families, caregivers, and interdisciplinary care teams
  • Provide oversight and guidance to clinical staff as needed to support quality patient outcomes

Qualifications
  • Active and unrestricted Practical Nurse (LPN) license in the state of Indiana
  • Minimum of two (2) years of home health nursing experience (required)
  • Valid driver's license and reliable personal vehicle
  • Demonstrated clinical knowledge and skills necessary to provide and coordinate patient care
  • Ability to assess patient data and identify appropriate care needs based on age and diagnosis
  • Strong communication, organization, and problem-solving skills
  • Proficient computer skills and experience with electronic documentation systems
  • Bilingual candidates strongly encouraged to apply
